If your child has a health condition that requires presciption medication to be administered at school (asthma, food allergies, bee allergies, diabetes, ADHD, or other chronic health conditions), you must complete the form below.

Also, if your child requires occasional adminstration of non-prescription medication, a form must be completed and returned to the office with the medication.
